Summary:
The Care Network Community Health Worker provides community based, comprehensive disease management/case management services for the chronically ill and socially complex.
Working closely with the CARE Network multidisciplinary team, the Community Health Worker provides case management support to program clients in the community/home/telephonic setting, as well as assistance and support with office/clerical functions as needed.
Responsibilities include helping clients navigate and access community services, other resources, and adopt healthy behaviors.
The CHW will provide job duties within the following track:
a) Navigation/Linkage to Resources:
CHWs will manage complex patient sub-populations, specifically patients experiencing homelessness.
Essential Functions:
Works collaboratively with the CARE Network team to help create linkages for the various health and social needs of patients, and find creative solutions to healthcare problems for at-risk patientsEstablishes rapport and trust with patients and builds relationship with patient's support system when availableProvides coaching activities related to life skills, self-management of chronic health conditions, and health care access skill buildingActs as a role-model in development of skills for interactions with healthcare system and social service agencies and personnelParticipates in patient goal-setting, care planning and care plan implementationCoordinates and arranges for needed services with appropriate local resourcesMaintains close working relationship with identified community-based providers, and participates in community-based partnerships that focus on care coordination and quality improvementProvides services to patients in a variety of settings, including but not limited to the hospital, patient homes, community sites, and provider officesProvides disease management education and coaching, focusing on individual client self-management principles.
This includes development of a personal health record, preparing for provider visits and community resource utilizationMakes appropriate and timely recording of interventions and patient progress in appropriate documentation systemsParticipates on committees or in other health-related activitiesProvides administrative assistance or other duties as directed by Care Transitions Program ManagerParticipates in CARE Network team activities related to program goal-setting and reports as neededAttends and participates in daily team huddlesAttends and participates in Department Staff meetingsProvides own transportation to community-based meetings and patient visitsReduce stigma and other barriers to initiating or continuing health care by providing necessary information to both community members and health care providersProvide navigation services to patients related to chronic diseases in coordination with clinical care teamSkills:
